Enrollment
A SEEDS Learning Center staff member will meet with each family to fill out necessary paperwork and provide families with a brief orientation to the program. Every child enrolled in SEEDS must provide written documentation showing that he/she is up to date on all his/her immunizations and vaccinations. The admission/enrollment packet ensures that we can provide the very best care to each child in the center. All forms satisfy the requirement of State Licensing Guidelines for record keeping. All forms will be reviewed monthly. If at any time there are changes to any of the information on any of the forms in the admission/enrollment packet, please notify the director to update your child’s records.
